JOB SUMMARY 

The nurse works in partnership with the family physicians and other members of the health care team to provide comprehensive primary health care to our patient population according to established standards and practices. The nurse focuses on providing assessment, screening, healthy lifestyle support, education and chronic disease management with a goal of improving health outcomes and facilitating access to services.

 

ESSENTIAL RESPONSIBILITIES

  • Triage and assess clients to identify appropriate nursing interventions.
  • Assess the immunization status of all patients and ensure immunizations are current.
  • Document accurate and pertinent patient information in a timely manner, including maintaining a comprehensive health summary or patient profile.
  • Provide and co-ordinate screening for health issues such as diabetes, hypertension, cholesterol, cancer screening, cardiovascular health, mental health, cognitive function and osteoporosis.
  • Utilize all health-care encounters as an opportunity to identify educational needs and provide health education (e.g., immunizations, screening, health lifestyle)
  • Monitor and maintain infection control practices according to established standards.
  • Advance primary care nursing through participation in professional development and practice development activities, mentoring students, orientating staff and participating in quality assurance initiatives.
  • Perform first-aid treatment on emergency cases; make the initial assessment and direct client to physician or nurse practitioner according to the degree of urgency.
  • Assist and/or perform medical procedures (e.g. ear irrigation, immunization, allergy shots, injections, IV therapy, dressing, suture removal, etc.) according to standards of practice.

 

ESSENTIAL QUALIFICATIONS

  • Registered Practical Nurse or Registered Nurse with the College of Nurses of Ontario
  • Current membership with the RNAO
  • Current First Aid and CPR certifications.
  • Bilingual (French and English)
  • Computer literacy: MS Word, Outlook, EMR and Internet.
  • The ability to plan, organize and prioritize, and foster a work environment conducive to the well-being of patients and staff.
  • Effective organizational, critical thinking, problem-solving and decision-making skills
  • Effective communication skills and ability to establish and maintain collaborative working relationships with team members and patients.
  • Minimum of one year of experience working in a family practice, community or primary care setting

Under the supervision of the manager, the Patient Services Clerks are responsible for the following:

  • Greet and register patients, as per patient registration guidelines;
  • Answer multi-line telephone, screen, and provide information/assistance or route caller to appropriate personnel, take messages and deliver to the appropriate personnel.
  • Schedule, confirm, cancel, and reschedule medical appointments, for physicians, AHP’s, and/or administration requests;
  • Establish and maintain confidential electronic medicals records; ensure accurate patient demographic information;
  • Perform entry-level billing and collection;
  • Other clinic related duties as required.

 

ESSENTIAL QUALIFICATIONS  

  • Must be eligible to work in Canada
  • Bilingual (French and English) essential;
  • Completion of secondary school or equivalent GED and a college program or other specialized courses for medical administration;
  • Computer literacy: MS Word, Outlook, EMR;
  • Some experience in a medical environment;
  • Knowledge of medical terminology an asset;
  • Strong attention to detail;

The primary focus of the Chiropodist will be to work to help patients eliminate foot pain, gain mobility and maintain healthy feet. The chiropodist is part of the clinic’s Diabetes chronic disease management program.

Qualifications:

  • Bachelor’s or Master’s degree in Physiotherapy;
  • Registration with College of Physiotherapy of Ontario;
  • Experience with supervision of support staff;
  • Experience in working with the geriatric population and chronic diseases;
  • CPR certification;
  • ADP certified, or willingness to become certified considered an asset;
  • Strong physical assessment & treatment planning skills;
  • Excellent time management and organizational skills;
  • Ability to work independently, yet collaboratively as a member of a team;
  • Excellent oral and written communication skills;
  • A commitment to best practices, quality, and client-centered care;
  • Bilingual (French and English)

Responsibilities:

As a member of our team you will be responsible for:

  • Initiating and managing the assessment and treatment for restoring movement/function, orthopaedic conditions and pain management;
  • Providing physiotherapy to outpatient residents in long-terms care, retirement home and assisted living setting
  • Developing a personalized treatment program that includes but is not limited to exercise, manual therapy and home exercise program.
  • Prescribing and educating clients utilization of mobility aids, assistive devices and other equipment;
  • Overseeing Physiotherapy Support Personnel (PTA).

Date of Commencement: As soon as possible.
