To send a copy of your medical records to another facility, including the University of Ottawa’s Student Health and Wellness Centre, the ByWard FHT requires your written authorization to release your medical records. Note that your medical records will remain in the custody of ByWard FHT for a period not less than 10 years, as required by law.
